I've seen this error when using a java script step to modify an existing value, like
Counter = Counter + 5
Then, in the lower part of the step, you select the variable, but forget to set the "Replace" parameter to yes. In this case, the "Counter" variable won't change, and the new value will be stored in "Counter_1".
