In either case, the destination can be changed - either to a specific place, or to limit it to specific individuals (in which case the content is associated with your personal area), or it lives at the 'top level'. In the latter case you can find the content either by searching for it, or by going to your profile and looking on the content tab.
Yes! If you are in a group or a space and start a discussion or ask a question, that is where the question or discussion will post.
If, on the other hand, you start a discussion from "My Dashboard" and click "Create" a discussion or document, you will have to decide where you want that content to be posted.
You can post in your own content, or you can select a space.
It seems that if you start from a group or a space the destination is the same group or space by default. However it is still possible to post somewhere using the options at the bottom of the screen (In a Place - Change / Specific People / The HDS Community) :
On the other hand you must specify a destination indeed when starting from the Dashboard, because there is no place selected.
