What is the recommendation for paths per lun?
Is it a 1 host port --> 1 storage port mapping or can this be a 1 wwn to 2 FA port ratio?
It all depends on Queue depth calculation and fan-in / fan-out ratios.
I would suggest to start with 1 wwn to 1 FA port to start with and keep it under monitoring (esp response time). When you feel like adding more FA ports, go ahead.
It all depends on planning & performance monitoring. There is no recommendations as such.

Each Host HBA should Only see 1 instance of each LDEV.
NEVER allow the same HBA to see more than 1 instance of any LUN.
An HBA can see multiple storage ports but, then you should divide the LUNs among those ports.
